POGO Satellite Clinics will be responsible for providing the specialized childhood cancer program with source documentation in a timely manner for all lab work, history, physical examination and medication administration and adherence. This documentation will be stored in the patient’s records, housed at the specialized childhood cancer program. The specialized childhood cancer program is responsible for the completion of all study Case Report Forms (CRFs). POGO Satellite Clinics will not enter any data onto study CRFs.
