4.1.3 Criteria for low-risk designation. Risk categorization refers to risk of bacteremia and serious complications, including mortality.
High-risk (any factor listed below)

- history of overwhelming sepsis within the previous 6 months
- age < 12 months
- Down Syndrome
- Bone marrow transplant patients within 6 months of transplant or at any time post transplant while still receiving immunosuppressants
- diagnosis of:
- acute myelogenous leukemia
- Burkitt lymphoma or leukemia
- acute lymphoblastic leukemia (ALL) in induction, consolidation or reinduction/delayed intensification phases
- advanced stage anaplastic large cell lymphoma
- high risk neuroblastoma
- relapsed leukemia or
- progressive/relapsed malignancy with bone marrow involvement
- presented with any of the following:
- sepsis syndrome*
- hypotension
- tachypnea
- hypoxia (O2 saturation < 94% on room air)
- new infiltrates on chest X-ray
- altered mental status
- severe mucositis
- vomiting
- abdominal pain, or
- evidence of significant local infection (e.g. tunnel infection, peri-rectal abscess, cellulitis)
