Workshop D – Preparing Survivors for Transition

Presentation Description: Successful transition for the young adult survivor of childhood/adolescent cancer requires preparation of the survivor, family and health care providers beginning well in advance of the event.  As a health care provider, having a fundamental understanding of transition and normal transition benchmarks is essential.  Additional topics needing to be addressed include medical information about prior cancer treatment, current health and future risks; when and how to access appropriate care; and the importance of education, employment and interpersonal relationships.  Awareness of age-associated risk-taking behaviours and reinforcement of appropriate health promotion is indicated.  This workshop incorporated brief presentations and encouraged attendee participation in discussions aimed at enhancing practice in health care transition.
