Description:Brockville and District Hospice Palliative Care Service promotes quality of life for people living with a life-limiting illness. The team offers assessments, counseling, pain and symptom management, comfort measures, companionship, respite and transportation. A Community Day Hospice Program offered weekly provides social interaction, therapies, activities and nursing consultations for patients. Bereavement support is provided through support groups, or individual counseling for those who have experienced the death of a loved one.
Eligibility:Clients and families in the Counties of Leeds and Grenville who are coping with a life-limiting illness.
Referral:Health Care providers, patients and/or family may make referrals to any service with the exception of hospitalized patients who require a physician referral.

Description:Emilys House is dedicated to caring for children with complex needs while supporting their families. It brings together high quality, respite and palliative care with the warmth and comfort of a child-focused environment. Emilys House can accommodate a maximum of 10 children at a time. However, capacity may vary at any given time based on the unique needs of patients and their families.
Eligibility:0-19 years of age, have a valid Ontario Health Insurance Plan (OHIP), and have a progressive life-limiting illness or major medical disease that demonstrates a progressive decline.
Referral:Referral can come from a parent/guardian, pediatric palliative care physician, family physician, nurse practitioner, nurse, social worker, or case manager.
Fee:Families are responsible for providing the childs medications, diapers and other supplies. Emilys House will cover supplies related to advanced respite care as well as all direct costs of care.
Description:ErinoakKids provides a comprehensive range of family-centred treatment, rehabilitation and support services to children with disabilities and their families who reside in Dufferin, Halton, Peel, Waterloo and Wellington - the Ministry of Children and Youth Services (MCYS) Central West Region of Ontario. The Ontario government funds the mandatory services offered at ErinoakKids through its Ministry of Children and Youth Services.
Eligibility:Children and youth (up to age 19) with multiple disabilities, complex disorders and speech delay living in Halton, Peel and, in some cases, Waterloo, Wellington and Dufferin.

Description:The Hospice Palliative Care Ontario (HPCO) represents and assists the development, education and enhancement of community hospice services including those services for children. Eligibility:Eligibility criteria includes: referral to CHEO Pediatric Palliative Care Outreach team, children/youth up to their 19th birthday, Ontario resident with valid OHIP coverage, child/youth has life-limiting illness with progressive decline in health status, family and child/youth must be willing to confront the challenging issues around a child's/youth's illness and death with the help of the interdisciplinary team.

Description:This program, offered by the Ontario Ministry of Community and Social Services, helps families with children who have a developmental or physical disability pay for special services in or outside the family home such as hiring professionals to help the child learn new skills and abilities, and provide respite support to the family.
