It Takes a Village: Optimizing Care for Children with Advanced Cancer

Presentation Description: Due to the wide geographical spread of patients with an oncologic diagnosis, children with advanced cancer in Ontario, and their families, often require careful coordination of multiple health care providers to optimize the delivery of palliative care.  This presentation will use a recent case as an example of a coordinated effort between multiple health care teams, over multiple sites of care, to highlight the challenges, successes and lessons learned from this privileged journey with a remarkable family.
