MedicAlert is Canada's largest member-based charity. The program offers a customized identification bracelet or necklet with an emergency hotline linked to your child's medical record.<br>The Canadian MedicAlert Foundation provides service to all of Canada in both English and French.

Available for:	Any Canadian with a medical condition or special need.
Referral:	Forms available through doctors, clinics, and pharmacies.
Application:	Apply by mail, phone, or Internet.
Hours:	Mon to Fri; 8:00 a.m. to 7:00 p.m.
Decision Time:	Immediate.
