POGO Recognition Awards - Call for Nominations

The Pediatric Oncology Group of Ontario (POGO) began a Recognition Program in 1999 to acknowledge significant contributions to the field of childhood cancer control. Three Awards are available, viz. POGO Companion Award, POGO Valued Contribution Award, and Friends of POGO Award. We write to invite your nominations for any/all of the Awards:

POGO COMPANION AWARD
The COMPANION AWARD was created to acknowledge significant contributions to the field of childhood cancer control over the course of a career, and to pay tribute to individuals (professional providers, researchers, etc.) who have made a prolonged and enduring, exceptional and sustained commitment to advancing state-of-the-art childhood cancer control. Recipients of this award have shown significant leadership in childhood cancer control, particularly in collaborative initiatives that sustain and advance the childhood cancer system within and also beyond the province. They have a history of participation in a wide range and increasing number of POGO initiatives over the years. The contribution of recipients exemplifies the following core values of POGO: excellence, leadership, innovation, integrity, collaboration and teamwork, and dedication. POGO’s COMPANION AWARD was first conferred in 2001.

POGO VALUED CONTRIBUTION AWARD
The VALUED CONTRIBUTION AWARD is given to individuals who have made a circumscribed, but substantial, contribution to the childhood cancer care network through the POGO mechanism. Recipients have contributed with commitment, enthusiasm and generosity over a prolonged period of time, in a way that exemplifies collaboration and teamwork and that transcends the requirements of their daily duties. Their contributions enhance childhood cancer control and the childhood cancer community and are typically made in the course of a career. The award was first conferred in 2002.

FRIENDS OF POGO AWARD
The FRIENDS OF POGO AWARD recognizes considerable and sustained contributions by valued partners of POGO to the collaboration’s overall vision, mandate and goals. Recognized contributions set a standard that exemplifies exceptional contributions from within the sector they represent. Typically, recipients will be individuals outside the professional caregiver network (e.g. strategic, political, financial and advocate partners); their contributions may be strategic, political, and/or financial, etc. and their participation has occurred consistently over a number of years and at a very high level. The FRIENDS OF POGO AWARD was conferred for the first time in 2008, POGO’s 25th anniversary year.


MAKING A NOMINATION and SUBMISSION REQUIREMENTS
Nominations can be made by submitting a nomination package to the POGO office, to the attention of Helen Craig. Each nomination package must include:
• Completed and signed nomination form - please download the Recognition Awards - Nominations Form & Request for Supporting Documents (PDF)
• 2 Letters of support (one to be provided by the nominator, PLUS one other), each clearly stating the award applied for AND specifically addressing the qualifications of the candidate, matched to the criteria of the award nominated for (please refer to award descriptions, above)
• Current curriculum vitae of nominee.


REVIEW PROCESS
Nominations will be accepted until 12:00 noon, Friday, September 17th, 2010, after which a committee, appointed by the POGO Board for this purpose, will review submissions. The nominator(s) and successful nominee(s) will be notified of the decisions of this group by 5:00 pm, Friday, October 15th, 2010. POGO will not necessarily make an award in each category every year.


RECOGNIZING EXCELLENCE
Successful candidates will be announced at the 2010 POGO Symposium Celebratory Dinner on Friday, November 19, 2010. They will receive a certificate of recognition and a commemorative gift from POGO. The selection committee may, in addition, propose that the names of some POGO Companion honourees be submitted for other awards, e.g. the Order of Ontario.
