2012 AfterCare Education Day - Registration Information

To register for AfterCare Education Day, please complete the registration form and submit electronically to POGO before January 20th. Upon POGO’s receipt of the form, you will be registered for the conference and will receive a confirmation via e-mail.

POGO receives some funding from the Ministry of Health and Long-Term Care (MOHLTC) to provide this educational opportunity. For those actively involved in the care of childhood cancer survivors in Ontario, the event is provided free of charge.

Other health care providers who do not work directly with the childhood cancer survivor population are invited to attend if the topics of the day are of interest, but there will be a minimal registration cost.
